Note: Our 14-year-old automated order form has just failed, it's under repair, so for the moment here's the old, by-hand way to do it!! It may actually be better...;-)

Just follow this three-step process to order online:

Select reports from the check box list.
Fill out your billing & shipping address/information below. Be sure to include the birth information.
On PCs, choose print "Cntrl-P" and then Microsoft print to PDF, which should save the whole page top to bottom. Save that and send the .pdf to us at townley@astrococktail.com. Macs have a similar arrangement. Just make sure both top and bottom of the form (2pp on some screens) get copied and check marks, other info are clearly there.
